I had to change 'HELO' to 'EHLO' on lines 531 and 536. The usage-example.php worked perfectly when I did that though. That was on an Apache server.

I am sorry I didn't noticed your comment earlier.

Today I released 6th version with auto detecting of "ESMTP" signature/response from server in order to correctly use
"EHLO" / "HELO" commands.
